在四环素类药物中,最敏感的是替加环素,其次为米诺环素和多西环素。替加环素是第一个用于临床的新一代的甘氨酰四环素,其作用机制是与细菌30S亚单位核糖体结合,阻止tRNA与核糖体的结合,抑制蛋白质的合成而杀菌。在米诺环素(二甲胺四环素)9位上添加N,N-二甲基甘氨酸,使得替加环素与核糖体的结合能力为米诺环素或四环素的5倍,因此替加环素有广谱抗菌活性。在体外试验中替加环素对鲍曼不动杆菌的抗菌活性大于亚胺培南,对亚胺培南耐药的鲍曼不动杆菌同样有效XX用替加环素对多重耐药菌的体外抗菌活性进行了研究,37MDRAB对替加环素的敏感率为97.3%,MIC90值为2 mg/L,其中16株耐美罗培南的鲍曼不动杆菌对替加环素的敏感率仍为100%,替加环素对鲍曼不动杆菌的MIC是米诺环素的1/8。有报道发现使用替加环素及其它院内感染控制措施使鲍曼不动杆菌的清除时间缩短Ellen S Moland对替加环素联合用药研究发现,替加环素与阿米卡星联用有杀菌作用及协同作用,可增加阿米卡星的药效。然而,高水平耐药的替加环素已经在一些多药耐药的不动杆菌中检出,并已知与染色体介导的外排泵上调有关。

 

Among tetracyclines, tigecycline is the most sensitive, minocycline and doxycycline are the second most sensitive. Tigecycline is first new generation of glycylcycline used in clinic and its action mechanism is to bind bacteria 30S subunit ribosome to prevent the binding of tRNA and ribosomes, restrain the protein biosynthesis and thus kill the bacteria. Add N, N-dimethyl glycine on the ninth site of minocycline (minocycline) to enable the binding ability of tigecycline and ribosomes to be 5 times of that of minocycline or tetracycline, therefore, tigecycline has broad-spectrum antibacterial activity. In the in vitro test, the antibacterial activity of tigecycline to acinetobacter baumannii is higher than that of imipenem and tigecycline is also effective to acinetobacter baumannii which is resistant to imipenem. In the research done by XX etc. on the in vitro antibacterial activity of multi-drug resistant bacteria by using tigecycline, the susceptibility rate of 37 MDRAB strains to tigecycline is 97.3, the MIC90 value is 2 mg/L, among them, the susceptibility rate of 16 meropenem-resistant acinetobacter baumannii strains to tigecycline is 100 and the MIC of tigecycline to acinetobacter baumannii is 1/8 of that of minocycline. It is reported that the removal time of acinetobacter baumannii can be reduced by using tigecycline and other nosocomial infection control measures. It is found in the study conducted by Ellen S Moland etc. on combined medication of tigecycline that the combined use of tigecycline and amikacin has bactericidal effects and synergistic effects and can increase the efficacy of amikacin. However, high-level tigecycline resistance has been detected in some multidrug resistant acinetobacters and it is known to be related to the up-regulation of chromosome-mediated drug efflux pump.
